I agree to the following terms and conditions - Must be at least 21 to play. - This is a family friendly event so kids are welcome to watch. - A minimum of 10 people and max of 14 to field a team. 4 females on field at all times. - All games are 7 innings or 50 minutes - whichever occurs first. - Prizes awarded to first and second place. - One person per registration. - Please don't forget read the liability waiver and sign and date the registration! This is an Amateur Athletic Waiver and a Release of Liability. In consideration of being allowed to participate in Kick for Kids, 1. I agree that prior to participating, will inspect the grounds and equipment to be used and if I believe anything is unsafe, will immediately advise my coach or supervisor of such conditions and refuse to participate. 2. I acknowledge and fully understand that I will be engaging in activities that involve risk of serious injury, including permanent disability and death and severe social and economic losses, which might result from, not only my actions, inaction or negligence of others, but from the rules of play, the condition of the premises or any equipment used. Further, that there may be other risks not known to me or not reasonably foreseeable at this time 3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death. 4. Along with my family, spouse and my heirs, release, waive, discharge and covenant not to sue the organizer, agents, affiliates, other participants, sponsoring agencies, sponsors, team coaches and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as "releasees" from demands, losses or damages on account of injury, including death, or damage of property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise. 5. Am of good physical condition to play kickball in an ever-changing outdoor setting on natural grass and/or dirt. I currently have health insurance and will maintain the health insurance while playing kickball or any other activities organized or hosted by the organizer.
